The FREC 4 course is an advanced pre-hospital care qualification designed for individuals operating in higher-risk, remote, or specialist environments. It builds on FREC 3 and focuses on enhanced patient assessment, advanced trauma care, and clinical decision-making in situations where access to emergency services may be delayed.

This qualification is suitable for those working in private medical provision, security and hostile environments, offshore and remote locations, and those progressing toward higher pre-hospital or clinical roles.

What the course covers

• Advanced roles and responsibilities of the First Responder
• Enhanced scene management and dynamic risk assessment
• Advanced patient assessment including full primary and secondary surveys
• Advanced airway management techniques and adjuncts
• Oxygen therapy and respiratory emergencies
• Catastrophic haemorrhage control including advanced bleeding management
• Shock recognition and advanced management strategies
• Advanced trauma care including chest injuries, spinal trauma, and multi-system trauma
• Management of fractures, dislocations, and musculoskeletal injuries
• Head injuries and altered levels of consciousness
• Medical emergencies including cardiac conditions, asthma, anaphylaxis, diabetes, seizures, stroke, and sepsis awareness
• Casualty monitoring and clinical decision-making
• Communication and structured handover to emergency services
• Record keeping, patient documentation, and legal considerations
• Infection prevention and control
• Managing patients in prolonged care or delayed evacuation scenarios

Assessment

Assessment is conducted throughout the course and includes:
• Practical skills assessments
• Written knowledge assessments
• Scenario-based simulations reflecting real-world environments

Certification

Successful candidates will receive the Qualsafe Level 4 Certificate in First Response Emergency Care (RQF), valid for 3 years.
